Certified or registered mail requires a signature if you request it. This is one way to get confirmation and proof that you opted out, In the event they refuse or neglect the confirmation mailing. Alternatively, you can postmark the letter at the post office window and ask for a receipt.

on March 10, the judge approved the request that only the last 4 of an individuals SSN be needed on the opt out forms. If you are adamant about not including your SSN last 4 digits, you could get it notarized at your cost and await a response with no guarantee.

If you do not include your social security number and mail it without, but are clear you want to opt out, you may receive correspondence to let you know it could not be accepted without it before you are opted in. (per CPT support representative on phone 3/7/2022)
